If you have never been to our clinic and have scheduled your first appoinment, you can speed up your first visit by filling out our intake form before you show up. You can fill out this form in advance by following these simple steps:

2) Print out the form on your printer

3) Fill out the form with a pen or pencil

4) Bring the completed form with you to your first visit
